Healthfirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists, coordinating refills, and supporting MTM services to improve adherence. You will communicate with patients and providers, document care, and partner with pharmacies to integrate treatment plans while ensuring compliance with regulatory standards.

Competenze

High School diploma or GED required.
Experience communicating with patients, providers, and pharmacists.
Nationally certified Pharmacy Technician or licensed/registered Pharmacy Technician.

Mansioni

Review medication lists to identify barriers and support adherence as Pharmacy Navigator.
Obtain refills, manage new prescriptions, and arrange appointments for members.
Coordinate MTM interventions with Healthfirst pharmacists and partners.
Monitor member medication adherence and coordinate with community pharmacies.
Conduct patient health assessments to identify barriers to adherence.
Handle member and provider communications by phone or in person when needed.
Perform additional duties as assigned.
