Job summary

HealthFirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists to improve adherence, reduce gaps in therapy, and prevent medication-related adverse events. You will facilitate refills and new prescriptions, arrange appointments, assist patients facing financial barriers, and coordinate MTM interventions with HealthFirst pharmacists and external partners.

The role requires strong communication with patients and providers; bilingual abilities preferred;

Qualifications

• High School Diploma or GED from an accredited institution.
• Proven written and verbal communication skills.
• Experience communicating with patients, providers, and pharmacists.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/pharmacist technician in good standing.

Responsibilities

• Review medication lists to improve adherence and prevent adverse events as a Pharmacy Navigator.
• Facilitate refills and new prescriptions, set up appointments, and assist with access to medications for financial barriers.
• Coordinate MTM interventions with HealthFirst pharmacists and external partners.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications by phone or in person.
• Perform additional duties as necessary.
