HealthFirst is seeking a Pharmacy Technician MTM Navigator to review medication lists, remove barriers to therapy, and ensure adherence. You will coordinate with pharmacists and external partners to integrate MTM interventions into broader healthcare management services.

Responsibilities include facilitating refills and new prescriptions, contacting providers, and communicating with members to support access to medications, including for those facing financial barriers.

Qualifications

• Requires HS diploma or GED.
• Experience communicating with patients, providers, and pharmacists.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/pharmacist technician in good standing.

Responsabilités

• Review medication lists to improve adherence and reduce gaps in therapy.
• Assist with refills, new prescriptions, appointments, and access to medications for financial barriers.
• Coordinate MTM interventions with HealthFirst pharmacists and external partners.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence.
• Handle member and provider communications by phone; in-person as needed.
• Perform additional duties as assigned.
