Job summary

Healthfirst is seeking a Pharmacy Technician MTM Navigator to review medication lists, eliminate barriers to therapy, and promote proper adherence.

You will assist members with refills and new prescriptions, arrange appointments, and coordinate MTM interventions with internal and external partners.

The role requires strong communication skills with patients and providers, a valid pharmacy technician certification, and a commitment to regulatory compliance.

Qualifications

• High School Diploma or GED is required.
• Experience communicating with patients, providers, and pharmacists about medications.
• Nationally certified Pharmacy Technician (PTCB, NHA) or licensed/registered Pharmacy Technician is required.

Responsibilities

• Review medication lists to support adherence and prevent adverse events.
• Accumulate refills, facilitate new prescriptions, schedule appointments, and assist with access to medications for patients with financial barriers.
• Coordinate MTM interventions with Healthfirst pharmacists and partners within healthcare management services.
• Monitor and communicate member medication adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence and gather medication histories.
• Handle member and provider communications by phone or in person as needed.
• Perform additional duties as necessary.
