HealthFirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists and eliminating barriers to therapy to promote adherence and prevent adverse events. You will obtain refills, set up appointments, and coordinate access to medications for patients facing financial or access challenges, while documenting care delivered.

Qualifications

• High School Diploma or GED required.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified or licensed Pharmacy Technician (PTCB/NHA) in good standing.

Responsabilités

• Review medication lists to ensure adherence and minimize adverse events.
• Facilitate refills and new prescriptions and coordinate access to medications.
• Communicate treatment plans with pharmacists and other providers to coordinate MTM interventions.
• Monitor member medication adherence with community pharmacies.
