Job summary

Carle Health is seeking an experienced oncology pharmacist to deliver comprehensive pharmaceutical care to oncology patients within the Carle Cancer Center. You will support specialty care across hematology/oncology, urology, ophthalmology, gynecology, and clinical trials, including MTM, dosing guidance, and safe chemotherapy preparation.

You will participate in rounds, collaborate with the hematology/oncology team, and help develop policies to promote safety and optimal therapy for cancer

Qualifications

• Licensed Pharmacist – Illinois IDFPR (certified/licensed in good standing).
• Bachelor’s Degree in Pharmacy is required.
• Clinical work experience is preferred.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices within the facility.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Function with hematology/oncology and inpatient pharmacy teams, with inpatient and outpatient responsibilities.
• Prepare investigational and standard of care medications for clinical trials.
• Attend rounds and provide pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
• Collaborate with hematology/oncology providers to enhance patient-centered care.
• Provide safe and effective drug therapy management; review adverse effects.
• Monitor adherence and toxicities for oral chemotherapy; oversee preparation of chemotherapy.
• Assist with transitions of care and staff development; precept students and residents.
• Develop medication policies and safety initiatives for oncology patients.
