Carle Health is seeking an experienced oncology pharmacist to deliver comprehensive pharmaceutical care for oncology patients and support specialty care across Urology, Ophthalmology, Gynecology, Research, and Expanded Care.

Qualifications

• Licensed Pharmacist – IDFPR (Illinois)
• Bachelor’s Degree in Pharmacy required
• Clinical work experience in hospital/oncology setting

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology and inpatient pharmacy teams on inpatient and outpatient responsibilities.
• Facilitate safe and timely preparation of investigational and standard of care medications for clinical trials.
• Attend hematology/oncology rounds and provide pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
• Collaborate with hematology/oncology providers to enhance patient-centered care.
• Provide safe and effective drug therapy management to patients.
• Review, monitor and manage adverse effects.
• Monitor care plans for regimen-specific adherence and toxicities to oral chemotherapy.
• Oversee and direct the preparation of chemotherapy.
• Assist with transitions of care and support clinical trial protocols.
• Develop medication-related policies, procedures and protocols and promote safety initiatives for oncology patients.
• Participate in pharmacokinetics dosing service activities and hospital committees
