Job summary

Carle Health’s oncology pharmacy team seeks a licensed pharmacist to deliver comprehensive pharmaceutical care to oncology patients, supporting hematology/oncology, urology, ophthalmology, gynecology, and research services across inpatient and outpatient settings.

You will participate in chemotherapy preparation, MTM, dosing recommendations, adverse event monitoring, and collaboration with clinicians to optimize therapy, while contributing to trials and safety initiatives.

Qualifications

• Licensed Pharmacist licensed by IDFPR.
• Bachelor’s Degree in Pharmacy required.
• Clinical work experience required.

Responsibilities

• Direct patient care to improve prescribing, administration, and monitoring practices.
• Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for both inpatient and outpatient care.
• Prepare investigational and standard-of-care medications for clinical trials.
• Attend hematology/oncology rounds and provide pharmacy services in the interdisciplinary practice.
• Develop medication-related policies, procedures and protocols and promote safety initiatives.
