Job summary

Carle Health is seeking a Pharmacist to deliver comprehensive pharmaceutical care to oncology patients within the Carle Cancer Center. You will serve as a clinical expert, supporting specialty care across Urology, Ophthalmology, Gynecology, Research, and Expanded Care.

You will participate in hematology/oncology rounds, collaborate with providers, oversee chemotherapy preparation, and support trials and safety initiatives as part of a multidisciplinary team.

Qualifications

• Certifications: Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR).
• Education: Bachelor‘s Degree: Pharmacy (Required).
• Work Experience: Clinical.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education.
• Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for inpatient/outpatient care.
• Prepare investigational and standard of care medications for trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Collaborate with providers to enhance patient-centered care.
• Ensure safe and effective pharmacotherapy and monitor adverse effects.
• Oversee chemotherapy preparation and transitions of care.
• Support clinical trial protocols and staff development.
