Job summary

Qualifications

• Licensed pharmacist with Illinois IDFPR certification.
• Bachelor’s degree in Pharmacy required.
• Clinical pharmacy experience in oncology settings preferred.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM and patient education.
• Dosing recommendations based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare investigational and standard medications for clinical trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
