Job summary

Qualifications

• Licensed Pharmacist with IDFPR license.
• Bachelor’s degree in Pharmacy.
• Clinical work experience in oncology or hospital setting.

Responsibilities

• Provide direct patient care to improve prescribing and therapy management.
• Perform MTM services and patient education as defined in applicable regulations.
• Recommend dosing based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare investigational and standard of care medications for trials.
• Attend rounds and contribute to interdisciplinary care at Carle Foundation Hospital.
• Assist with transitions of care and protocol adherence.
• Develop and promote medication safety policies and procedures.
