You will serve as a clinical expert supporting Urology, Ophthalmology, Gynecology, Research, and Expanded Care, participating in rounds and clinical trials.

Qualifications

• Licensed Pharmacist in Illinois (IDFPR)
• Bachelor’s Degree: Pharmacy required
• Clinical work experience in a hospital/oncology setting

Responsibilities

• Direct Patient Care: intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): provide MTM services and patient education as defined by state act.
• Dosing Recommendations: determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
• Prepare investigational and standard of care medications for clinical trials.
• Attend rounds and provide pharmacy services in the interdisciplinary team.
• Assist with transitions of care and staff development.
• Develop and promote oncology medication policies and safety initiatives.
• Oversee chemotherapy preparation and participate in committees as needed.
