Qualifications

• Licensed Pharmacist (IDFPR)
• Bachelor's Degree in Pharmacy required
• Clinical work experience

Responsibilities

• Direct patient care to improve prescribing and monitoring.
• Provide MTM services and patient education.
• Determine optimal dosing based on age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology for inpatient and outpatient care.
• Assist with chemotherapy preparation and investigational trials
