Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with clinical documentation. You will observe visits, document encounters in real time, and assist with coding and care coordination in a fast-paced clinic setting.

The role emphasizes accurate documentation, collaboration with healthcare teams, and onboarding into our value-based care model. This is a full-time, in-clinic position with opportunities to grow in health informatics and coding.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering (strongly preferred).
• Scribe or transcription experience is a plus.

Responsibilities

• Document patient encounters in real time (approximately 80%).
• Observe the provider in the exam room and assist with documentation.
• Record structured notes including history, assessment, plan and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries and support care team with documentation improvements.
• Support daily huddles and clinical documentation reviews.
• Place orders and referrals; assist with administrative tasks as needed.
