Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time and assisting with CPT/ICD-10 coding. You will participate in morning huddles, observe visits, and help with post-visit documentation.

The role emphasizes value-based care, chronic disease documentation, and coordination within a care team. Ideal candidates have medical terminology knowledge, clinical exposure, and strong typing and computer skills.

Formación

• Knowledge of medical terminology and common medications.
• Prior clinical experience, shadowing or volunteering.
• Prior scribe or transcription experience preferred.
• Advanced listening and communication skills are required.
• Ability to learn new documentation workflows and CPT/ICD-10 coding.
• Fluency in Spanish, Polish, Russian, or other community languages is valued where indicated.

Responsabilidades

• Documenting patient encounters in a structured note, including history, assessment, plan, and exam.
• Assign CPT and ICD-10 codes and prepare after-visit summaries.
• Support clinical documentation improvement with provider collaboration.
• Provide administrative support to the provider and care team.
