Qualifications

Responsibilities

• Document patient encounters in real time and prepare structured notes
• Join providers in exam rooms to observe visits and document findings
• Assign CPT and ICD-10 codes accurately
• Prepare after-visit summaries
• Support clinical documentation improvement and review patient records
• Coordinate with care team for documentation-related tasks
