Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time, observing visits, and ensuring precise clinical notes, ICD-10 and CPT coding. You will join providers in the exam room and help prepare After Visit Summaries.

This paid, full‑time role offers on‑the‑job training in value‑based care, chronic disease documentation, and health informatics.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience including shadowing and/or volunteering.
• Prior scribe or transcription experience (preferred).

Responsibilities

• Document patient encounters in a structured note with history, assessment, plan, and exam.
• Assign CPT and ICD-10 codes and prepare After Visit Summaries.
• Support clinical documentation improvement and review medical records.
• Provide administrative support and assist the care team with documentation-related tasks.
