Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time clinical documentation. You will observe patient visits, document encounters, assign CPT and ICD-10 codes, and help prepare after-visit summaries.

This role emphasizes accuracy in documentation, knowledge of value‑based care, and collaboration with the care team. A 1‑2 year commitment is encouraged; full-time in clinic with in-person duties.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering, strongly preferred.
• Prior scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note including history, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries and support daily huddles and Clinical Documentation Reviews.
• Provide administrative support to the care team and other duties as assigned.
