Your Health in Georgia is seeking a Clinical Coordinator to support our primary care team. The role involves preparing charts, coordinating patient visits, and assisting providers while ensuring accurate documentation and privacy compliance.

The position is full-time, on-site, with typical 8:00 AM to 5:00 PM hours and daily office work in Gwinnett County and surrounding areas. Prior clinical coordination experience is valued in a fast-paced, value-based care setting.

Qualifications

• Must be a CMA or licensed nurse (LPN/RN) in the state of employment.
• Minimum 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider; update Navina as needed.
• Maintain confidentiality and privacy compliance; verify active consent forms.
• Reconcile medications.
• Collaborate with care team to schedule and coordinate patient appointments.
• Interact with patients via email, portal, phone, text, in person to gather information and provide instructions.
• Utilize communication tools such as phone, email, AthenaText, Teams.
• Accurately document and log care management activities.
