Job summary

Your Health Organization in Georgia is seeking a Clinical Coordinator to support patient care across facilities. The role focuses on chart preparation, scheduling, and care-management logging to ensure seamless patient care in a fast-paced value-based environment.

The position is full-time, 8 hours daily (8:00 AM–5:00 PM), working in-office, with travel between offices as needed to support providers and patients in Gwinnett County and surrounding areas.

Qualifications

• Must be CMA or licensed nurse licensed in the state of employment.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and sound judgment.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation, test results, diagnoses, and relevant information are accessible for the provider using Navina.
• Ensure confidentiality and adhere to privacy regulations when handling sensitive patient information.
• Reconcile medications.
• Collaborate with care team to schedule and coordinate patient appointments with the Company’s provider(s).
• Interact with patients via email, portal, phone, text, or in person to gather information and provide pre-appointment instructions.
• Utilize communication tools such as phone, email, AthenaText, Microsoft Teams.
• Document and log care management activities accurately.
