Your Health Organization in Georgia is seeking a Clinical Coordinator to support patient care in an in-office, full-time capacity. The role focuses on chart prep, appointment scheduling, and care-management logging to assist providers and patients in a value-based care setting.

Qualifications include CMA or LPN/RN licensure, CPR certification, and at least 1 year in a clinical environment. Expect daily travel between offices and facilities within Gwinnett County and surrounding areas as needed.

Qualifications

• Must be a CMA or licensed nurse in the state of employment.
• Minimum of 1 year healthcare experience preferred.
• Current CPR certification required.
• High school diploma or equivalent required.
• Strong communication and teamwork abilities are essential.
• Ability to read, write, and communicate effectively.

Responsibilities

• Review and organize patient medical records prior to visits.
• Ensure documentation, test results, and diagnoses are available for providers.
• Protect patient confidentiality and verify consent forms.
• Reconcile medications and coordinate patient appointments.
• Schedule visits and communicate pre-appointment instructions.
• Log care management activities and use communication tools.
