Job summary

Your Health Organization in Georgia is seeking a Clinical Coordinator to support patient care across our facilities. This full-time role handles chart prep, scheduling, care management logging, and cross-team coordination with providers.

Responsibilities include organizing records, ensuring documentation is complete in Navina, and communicating with patients via email, portal, phone, and text. A CMA or RN license with CPR is required; travel between offices is expected.

Qualifications

• Must be a CMA or licensed nurse and licensed in the state of employment.
• Minimum of one year experience in a healthcare setting.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Ability to work flexible hours and travel between offices/facilities.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and results are accessible for the provider (Navina).
• Maintain confidentiality and privacy compliance; verify consent forms in charts.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team members.
• Interact with patients via multiple channels to gather information and provide pre-appointment instructions.
• Document and log care management activities.
