Job summary

Your Health in Georgia is seeking a Clinical Coordinator to manage clinical needs, chart prep, scheduling, and care coordination across facilities. The role emphasizes accurate documentation, patient interaction, and support for providers and care teams.

The position is full-time, 8-hour shifts, Monday through Friday, with duties performed daily in our facilities and opportunities for professional growth within a value-based care model.

Qualifications

• Must be a CMA or licensed nurse in the state of employment.
• At least 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills under all conditions.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Must be able to read and communicate effectively.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Must be able to travel between offices/facilities.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and results are accessible for the provider (Navina usage).
• Maintain accurate up-to-date patient records with demographics and insurance details.
• Ensure confidentiality and privacy regulations are followed; verify consent forms.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, or in-person.
• Utilize communication tools (phone, email, AthenaText, MS Teams).
• Document and log care management activities.
