Job summary

Your Health Organization in Georgia is seeking a Clinical Coordinator to support patient care, chart preparation, scheduling, and care management activities. The role supports providers across Gwinnett County and surrounding areas in a busy primary care setting.

The ideal candidate will be a CMA or licensed nurse, with CPR certification and at least 1 year of healthcare experience, able to work Monday–Friday in an in-office, full-time position and travel as needed.

Qualifications

• CMA or licensed nurse in good standing with the appropriate board.
• CPR certificate valid.
• High school diploma or equivalent.
• At least 1 year healthcare setting experience preferred.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure provider has access to all relevant documentation and data.
• Maintain confidentiality and privacy regulations; verify consent forms.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Engage with patients via email, portal, phone, text, or in person to gather information and schedule visits.
• Use tools like Navina, AthenaText, and Teams for communication.
• Document and log care management activities.
