Your Health Organization in Georgia is seeking a Clinical Coordinator to support patient care across our facilities. The role involves chart preparation, scheduling visits, logging chronic care management activities, and providing provider and patient support.

The position is full-time and onsite, with standard hours 8:00 AM–5:00 PM, Monday–Friday. Candidates must be CMA or licensed nurses, possess CPR, and be able to travel between offices as needed.

Qualifications

• Must be CMA or licensed nurse in-state.
• CPR certification required.
• High school diploma or equivalent.

Responsibilities

• Review and organize patient medical records for visits.
• Ensure documentation and results are accessible for providers using Navina.
• Maintain confidentiality and verify active consent forms.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Communicate with patients via email, portal, phone, or text.
• Log and document care management activities.
