Job summary

Your Health in Suwanee, GA is seeking a Clinical Coordinator to support clinical needs of our patients. The role includes chart preparation, scheduling visits, managing chronic care activities, and supporting providers and patients.

This full-time, office-based position runs Monday through Friday, 8:00 AM – 5:00 PM, with travel between offices as needed and opportunity for growth within a fast-paced, value-based care environment.

Qualifications

• Must be CMA or licensed nurse in the state of employment.
• Current CPR certification and valid driver’s license.
• Experience in healthcare setting preferred.
• Proficient with patient records and scheduling systems.

Responsibilities

• Review and organize patient medical records prior to visits.
• Ensure documentation, test results, and diagnoses are accessible.
• Maintain up-to-date patient records including contact and insurance info.
• Protect patient confidentiality and manage consent forms.
• Reconcile medications as needed.
• Collaborate with care team to schedule patient appointments.
• Communicate with patients via email, portal, phone, or in person.
• Record and log care management activities accurately.
