Qualifications

Responsibilities

• Review and organize patient medical records for visits.
• Ensure documentation and test results are accessible for providers.
• Maintain patient confidentiality and verify consent forms are complete.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Communicate with patients via email, portal, phone, or in person to schedule visits and give pre-appointment instructions.
• Document and log care management activities.
