Qualifications

• Past experience in intake or medical office administration.

Responsibilities

• Oversee client onboarding and care plan management.
• Coordinate scheduling and communication with clients, caregivers, and internal departments.
• Document all client-related communications in the client profile.
• Review and update care plans within specified timelines.
• Notify supervisors promptly of changes in client status to ensure continuity of services.
• Maintain a clearly defined schedule for every active client on the coordinator’s dashboard.
• Record all client-related communications for accurate documentation.
