Job summary

Open Arms Solutions is seeking a compassionate Community Care Coordinator to deliver high-quality in-home care for seniors in the Chicago area. You will manage client care plans, guide intake and budgeting, and support a team of caregivers to ensure a seamless, personalized experience.

The role requires 3–5 years in a client-facing environment, CNA preferred, strong communication, organization, and problem-solving skills, and willingness to travel up to 50% for client visits.

Qualifications

• 3–5 years in a high-touch, client-facing role, preferably in senior care or hospitality.
• CNA certification strongly preferred.
• Proven care management experience and caregiver supervision.
• Excellent written and verbal communication with discretion.

Responsibilities

• Conduct client/family consultations to create tailored care plans addressing medical, functional and lifestyle needs.
• Guide intake, registration, care planning and budgeting for in-home services.
• Manage a caseload and serve as main point of contact for clients.
• Provide emergency coverage for client shifts when needed.
• Support hiring, training and mentoring of caregivers to maintain concierge-level service.
• Match clients with caregivers based on skills, personality, and preferences.
