• Share on-call duties every other month.
• Coordinate input of care plans and staffing with caregivers.
• Complete monthly client calls and document in files.
• Update care plans, upload reports, and communicate changes.
• Match caregivers to clients and ensure care plan compliance.
• Participate in staff training and meetings.
• Perform clerical tasks such as filing, uploading, copying, or faxing.
• Assume other duties as assigned.
