Serenity Home Health Care Agency LLC is seeking a Client Coordinator for our Chicago area team. The role involves managing clients throughout their lifecycle, handling initial intake calls, and providing information on services and fees.

You will maintain client and employee records, ensure quality of care, and support staff with scheduling and communications. The position requires strong customer service and communication skills, proficiency with computers, and the ability to work in English;

Qualifications

• Demonstrated customer service and communication skills.
• Problem solving and attention to detail expected.
• Fluent in spoken/written English; additional languages a plus.

Responsibilities

• Is the first point of contact for clients, industry partners, and care providers, delivering excellent customer service.
• Takes inquiries and gathers information to inform client care/plan and fees.
• Monitors dashboard daily; follows up with caregivers and clients.
• Shares on-call responsibilities on a rotating schedule every other month.
• Inputs care plans, ensures staffing, and briefs caregivers prior to care start.
• Completes monthly client calls and documents updates to files.
• Maintains care plan documentation and communicates changes to caregivers.
• Ensures best caregiver-client matches and compliance with plans.
• Assists with staff trainings and administrative duties as needed.
