Qualifications

• High school diploma or equivalent.
• 1–2 years client-facing experience in home care, healthcare, social services, case management or related field.
• Customer service experience demonstrating strong communication, empathy and problem-solving skills.

Responsibilities

• Oversee wellbeing, service coordination, and overall care of all assigned clients.
• Review new client profiles for accuracy and completeness.
• Complete staffing for all new clients within 48 hours; escalate delays to Branch Manager.
• Process updates to client care plans within 1 hour of receiving information.
• Notify supervisors promptly of changes in client status or care needs.
• Maintain a clearly defined schedule for every active client on the coordinator’s dashboard.
• Record all client-related communication in the client profile for accurate documentation.
• Conduct proactive outreach every Friday to clients with services on hold.
• Review missed visits weekly and schedule makeup hours.
• Close client profiles upon service termination after full investigation and documentation.
• Ensure uninterrupted services for IDOA and IDHS clients unless payers instruct otherwise.
• Begin renewal process for VA clients at least one month before authorization expiration.
• Ensure completion of monthly surveys for all clients under management.
• Ensure all caregivers serving clients complete an annual performance evaluation.
