Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care and support to patients in Wichita, KS. You will work under a Case Manager RN, perform daily living activities, review care plans, and assist with transfers while maintaining EMR records.

Qualifications

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• Current CPR certification preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
