Job summary

Moments Hospice is seeking a compassionate CNA to provide essential personal care and support on the road, partnering with an RN Case Manager to ensure patient comfort and dignity. You will perform daily living activities, review care plans, and help with transfers while documenting in EMR.

Qualifications

Certified Nursing Assistant (CNA) license (required).
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
Current CPR certification (preferred).

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN
