Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide personal care and support to patients in their homes. You will collaborate with an RN Case Manager, perform daily living activities, and help maintain electronic medical records while delivering high-quality patient care.

Ideal candidates have a valid CNA license, at least 12 months of caregiving experience, and strong communication skills.

Qualifications

• CNA license is required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• Current CPR certification is preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
