Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to support patients at home and on the road in Minnesota. You will perform personal care, review care plans, assist with transfers, and document in electronic medical records under the guidance of an RN Case Manager.

Qualifications

CNA license required.
12 months caregiving experience in relevant settings.
CPR certification preferred.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, etc.).
Review patient care plans.
Assist with lifting/transferring patients.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under RN Case Manager direction.

Education

Qualifications

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)

Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
