Job summary

Moments Hospice is seeking a compassionate CNA to join our on-the-road care team in Wisconsin Dells. You will provide essential personal care, support care plans with a dedicated RN, and help maintain patient comfort and dignity at the end of life.

Qualifications

• Certified Nursing Assistant (CNA) license is required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required.
• Current CPR certification is preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans with RN Case Manager.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
