Job summary

Moments Hospice in Minnesota seeks a Certified Nursing Assistant (CNA) to provide compassionate in-home care on the road. You will work under an RN Case Manager, perform personal care tasks, review care plans, and help patients maintain dignity in their final days.

Responsibilities

Assist with lifting/transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
