Job summary

Moments Hospice is seeking a dedicated Certified Nursing Assistant (CNA) to join our on-the-road care team. You will provide essential personal care, review care plans, and support patients with daily living activities under an RN Case Manager.

Qualifications

• Certified Nursing Assistant (CNA) license (required).
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
• Current CPR certification (preferred).

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
