Job summary

Moments Hospice in Minnesota is seeking a dedicated CNA to support patient care on the road. You will perform personal care, help review care plans, and work with an RN Case Manager to ensure comfort and dignity.

This position offers a competitive hourly wage, a company car with fuel and insurance, a company cell phone, and comprehensive benefits including health, dental, and vision insurance, plus PTO and a 401(k) plan.

Qualifications

CNA license required and at least 12 months caregiving experience in healthcare settings.
Current CPR certification preferred.
Valid driver's license and auto insurance required.

Responsibilities

Provide personal care (bathing, dressing, grooming, toileting, skincare).
Review care plans and assist with transfers.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under RN Case Manager direction.

Qualifications

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
