Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care, assist with daily living activities, and support an RN Case Manager on the road. You will perform bathing, grooming, mobility assistance, and maintain patient comfort in home settings.

Requirements include CNA license, 12 months caregiving experience, CPR preferred, valid driver's license and auto insurance. Benefits include health, dental, vision, 401(k), PTO, company car and cell phone.

Qualifications

• Certified Nursing Assistant (CNA) license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
• Current CPR certification preferred.
• Valid driver's license required and auto insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans and assist with lifting and transferring patients.
• Assist with range of motion, positioning, ambulation and dining needs.
• Maintain electronic medical records and work under RN Case Manager direction.
