Job summary

Moments Hospice seeks a Certified Nursing Assistant (CNA) to provide compassionate personal care under an RN Case Manager. You will perform daily living activities, maintain EMR records, and participate in training to advance your skills.

This field-based role involves on-road travel with a supportive team and comprehensive benefits. Ideal candidates hold a CNA license, 12 months caregiving experience, CPR certification preferred, and a valid Driver's license with auto insurance.

Qualifications

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
• Current CPR certification preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
