Job summary

Moments Hospice is seeking a compassionate CNA to perform personal care and support under an RN Case Manager. You will help patients with daily living activities, review care plans, and maintain EMR records, while upholding safety and professional boundaries in a home-care setting.

The role offers a company car, cell phone, and comprehensive benefits with a generous PTO plan. CNA license required with at least 12 months of caregiving experience; CPR preferred.

Qualifications

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
