Job summary

Moments Hospice in Green Bay, WI is seeking a compassionate CNA to support patients at home and on the road, under the supervision of an RN Case Manager.

Responsibilities include personal care, reviewing care plans, maintaining EMR, and assisting with mobility, nutrition, and daily living activities. Attractive benefits package and company car provided.

Qualifications

Certified Nursing Assistant license is required.
12 months of caregiving experience in Home Care/Assisted Living/Hospice etc. is required.
Current CPR certification is preferred.
Excellent customer service skills are expected.
Strong written and verbal communication skills are required.
Good time management and ability to work with electronic medical records.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
Review and follow patient care plans with RN Case Manager.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.

Qualifications

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)

Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
