Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our on-the-road team. You will perform personal care, review care plans, assist with transfers, and support patients to maintain dignity under an RN Case Manager.

Requirements include an active CNA license and 12 months of caregiving experience; CPR is preferred. We offer a competitive hourly wage, a company car with fuel, a cell phone, and comprehensive health benefits.

Qualifications

• Certified Nursing Assistant (CNA) license is required.
• At least 12 months of caregiving experience in home care, assisted living/memory care, skilled nursing, hospital or hospice is required.
• Current CPR certification is preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
