Job summary

Moments Hospice is seeking a compassionate CNA to join our on-the-road care team in Minnesota.

You will provide essential personal care, review care plans with an RN Case Manager, assist with transfers, mobility, nutrition, and daily living activities while maintaining accurate electronic medical records.

Qualifications

• Must have CNA license (required).
• Minimum 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
• Current CPR certification (preferred).

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
