Job summary

Racine County seeks a dedicated clinician to deliver psychosocial rehabilitative services to adults and adolescents with mental illness or substance use disorders in the community under a CCS plan. You will work with a multidisciplinary team, families, and community partners to support recovery and safety.

Responsibilities include creating recovery‑oriented plans, home visits, crisis intervention, coordination of resources, and thorough documentation in compliance with DHS 36 and Medicaid

Qualifications

• Bachelor’s degree in Human Services field required; Master’s degree preferred.
• Experience with treatment programs for adults/youth with mental illness or substance use disorder.
• Valid Wisconsin driver’s license and automobile insurance required.
• Prompt and regular attendance.

Responsibilities

• Formulate recovery‑focused treatment plans under supervision.
• Provide services to individuals and families in home and community settings.
• Coordinate agency staff and community resources to support treatment goals.
• Provide supportive counseling and crisis intervention as needed.
• Maintain a treatment‑oriented environment and ensure objective outcomes.
• Provide medication management support as directed by physician.
• Complete medical/financial record documentation for billing.
• Discharge planning with client and clinical director.
• Attend staff meetings and trainings; adapt to flexible hours, including evenings.
