Job summary

Wisconsin Community Services seeks a compassionate Care Coordinator to support adults with mental health and/or substance use disorders within the CCS program. You’ll develop personalized recovery plans, collaborate with providers, and advocate for timely supports, while maintaining thorough documentation and ongoing client engagement.

The role requires strong communication, organizational skills, and a commitment to recovery and harm reduction.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field required; Master’s preferred.
• One year of experience working with individuals experiencing mental illness and/or substance use.
• Valid driver’s license, automobile, and insurance sufficient to meet agency requirements.
• Passion for mental health advocacy and belief in recovery and harm reduction models.
• Must meet all employee requirements including references, background checks, and acceptable driving record.

Responsibilities

• Build trusting relationships with individuals and families; meet patients where they are.
• Coordinate supports with a trauma-informed approach.
• Develop, implement, and update individualized recovery plans.
• Liaise with therapists, healthcare professionals, and case managers to ensure coordinated care; complete referrals as needed.
• Monitor progress toward goals; provide coaching and psychosocial rehabilitation.
• Maintain regular communication via in-person visits, calls, and follow-ups.
• Ensure access to transportation for program-related appointments.
• Respond to crises with empathy and professionalism; document all interactions.
• Complete county and agency-required trainings; maintain accurate records for CCS eligibility.
