Racine County in Wisconsin is seeking a qualified professional to deliver psychosocial rehabilitative services for adults and adolescents with mental illness or substance use disorders within the community. You will develop CCS plans and work with a multidisciplinary team to support client goals.

You will provide home visits, crisis support, and help with discharge planning while ensuring documentation meets DHS 36 and Medicaid standards. Evening shifts may be required.

Qualifikationen

• Bachelor’s degree in human services or related field is required; Master’s degree preferred.
• Experience with treatment programs involving adults and youth with mental illness and/or substance use disorder.
• Valid Wisconsin driver’s license and automobile insurance are required.

Aufgaben

• Formulate recovery-focused client-centered treatment plans for clients on caseload under supervision.
• Provide services to individuals and their families in homes and the community.
• Coordinate agency staff and community resources to support treatment plan goals.
• Provide supportive counseling and crisis intervention as needed.
• Maintain a treatment-oriented environment and ensure consistency in treatment objectives.
• Provide medication management support for client medication adherence as directed by supervisor.
• Discharge planning with client and clinical director assistance.
• Attend staff meetings and trainings; participate in supervision and case reviews.
• Maintain flexible work schedule, including potential evening shifts.
• Communicate information clearly per agency, DHS 36 and Medicaid standards.
• Make intervention decisions using Best Practice philosophy.
• Coordinate with law enforcement, families, and community resources for crisis resolution and referrals.
• Document health care/medical and financial records accurately.
