Job summary

Responsibilities

• Coordinate and monitor clinically relevant treatment plans with a multidisciplinary team.
• Advocate for community resources to support recovery in natural living environments.
• Document consumer contacts, timesheets and billing information accurately.
• Attend trainings, meetings and adapt to flexible schedules to meet client needs.
• Perform duties in a trauma-informed, person-centered, recovery-oriented framework.
